Once we have grouped the classes you will receive the class schedule for your child.PLEASE READ CAREFULLY:Participation Consent(Required) I, the parent/ guardian of the above-named child, hereby, consent to participating in the enrichment program. I understand that my child will be participating in physical activities. To the best of my knowledge, my child is physically fit and able to participate. Should it be necessary for my child to have medical treatment while participating in the enrichment program, I hereby give the school personnel or medical personnel permission to render medical treatment as necessary to my child. Furthermore, I do hereby waive, release, absolve, indemnify, and agree to hold harmless Pineview Preschools, its owners, officers, and staff from all claims arising out of any injury to my child for any cause as a result of a third-party liability and acts of God. Financial Authorization(Required) I authorize for Pineview Preschools to charge the enrichment program fees on my account through Tuition Express/ ACH. I understand this fee is non-refundable and non-transferable towards another student, towards tuition, or towards any other fees.● Ballet Uniform: Students must wear black leotard, pink tights and ballet slippers (tutus are not permitted). Please bring the ballet uniform every Tuesday.
