I, the parent/ guardian of the above-named child, hereby, consent to participating in the preschool summer camp sponsored by Pineview Preschools. I understand that my child will be participating in activities such as: water activities/ splash days, animal shows, outdoor activities such as soccer and other sports. To the best of my knowledge, my child is physically fit and able to participate. Should it be necessary for my child to have medical treatment while participating in the summer camp, I hereby give the school personnel or medical personnel permission to render medical treatment as necessary to my child. Furthermore, I do hereby waive, release, absolve, indemnify, and agree to hold harmless Pineview Preschools, its owners, officers, and staff from all claims arising out of any injury to my child for any cause as a result of a third-party liability and acts of God.
