I, the parent/guardian of the above-named child, hereby consent to my child’s participation in the Preschool Summer Camp sponsored by Pineview Preschools. I understand that camp activities may include, but are not limited to, water activities and splash days, animal shows, and outdoor activities such as soccer and other sports. To the best of my knowledge, my child is physically fit and able to participate in these activities. In the event that my child requires medical treatment while participating in Summer Camp, I hereby authorize Pineview Preschools’ personnel and/or licensed medical personnel to obtain and/or provide medical care as deemed necessary.
Furthermore, I waive, release, absolve, indemnify, and agree to hold harmless Pineview Preschools, its owners, officers, employees, and staff from any and all claims arising out of any injury to my child resulting from third-party actions or acts of God while participating in Summer Camp activities. By signing below, I acknowledge that I have read and fully understand this Consent, Medical Authorization, and Release of Liability, and that I voluntarily agree to its terms.
